Common questions

What forms do I need to fill out before my first visit?
Three: a medical and dental history form, a HIPAA notice acknowledgement, and a financial policy acknowledgement. We email these as PDFs after you book, or you can fill them out on a tablet when you arrive — about 10 to 15 minutes either way.
Can I fill out the forms online?
Yes — we will send you a secure link after you book. Filling them out at home gives Dr. Youn time to review your history before your visit, and gets you out of the waiting room faster.
What should I bring with me?
Your photo ID, your dental insurance card if you have one, a list of any medications you take, and any recent dental records or X-rays from a previous office (we can also request these on your behalf).
What if I do not have my old records?
No problem. Let us know your previous dentist and we will request your records and X-rays. If they are not available, we will take fresh digital X-rays at your first visit — they are usually covered by insurance.
